Sending in your Modem
Once your SRO number has been issued, send your modem to:

Payment Methods (if applicable)
If your modem requires Non Warranty repair, we will notify you
upon the issuing of the SRO.

We accept Visa, Master Card, Purchase Orders, Certified checks
and personal checks.  (Terms of payment differ outside the USA).
The modem will not be repaired until payment has been approved or
received.

Credit Card
To process your credit card order:

 *  Card number
 *  Exp date
 *  Card type - Visa or Master Card *only*
 *  Issuing bank
 *  Daytime phone number
 *  Shipping address (if different than billing address)


Purchase Order
The P.O. must contain:

 *  Company Logo
 *  P.O. #
 *  Ship to and bill to address
 *  Part numbers of parts being ordered
 *  Cost of repair or parts
 *  Quantity of parts
 *  Net 30 terms
 *  Tax Exempt # if applicable
 *  Signature and position of the signer
 *  SRO number

number or address on the following page.

Prepay
Once your SRO number has been issued, send a check (check
must be for $100.00 or less), Money Order or Certified Check
to the appropriate address or fax number on the following
page.
