Symptom:

Errors scroll on system console or can be found in /usr/adm/messages file.

Cause:

Device entry duplication in the /etc/inittab file.

Solution:

Edit the /etc/inittab file and remove duplicate device entrys. Even though the entry may not be set to 'respawn', any duplication of devices will generate the errors. Be on the lookout for both upper and lower case device entries.

After removing duplication, type init q from the UNIX root prompt, to re-read the inittab changes.
